Summary
This page outlines comprehensive post-hospital and post-rehab home care that supports a safe transition home and reinforces treatment plans. It details tailored assistance after joint replacement, cardiac procedures or events, and eye surgery—covering medication reminders, support for prescribed therapy and heart-healthy routines, meal preparation, errands/transport needs, and home safety. Caregivers also help reduce pneumonia risk by maintaining a clean environment and encouraging deep-breathing adherence, good nutrition, and hydration. Testimonials emphasize compassionate, professional teams who communicate well, adapt to changing needs, and provide dignified, reliable support for clients and families.
Types of After-Hospital Care
After rehab or hospital discharge, continuing support through home care after hospital discharge reinforces treatment to get an individual back to doing the things he or she loves. Our qualified caregiving teams are fully trained for the transition from the hospital or skilled rehab center to home and are ready to provide support every step of the way with comprehensive post-hospital care services and safe post-hospital care at home.
Examples of elderly care after hospital discharge and post-operative home care include:
Joint Replacement
Recovering from joint replacement surgery can make simple tasks challenging for several weeks, potentially even months. Post-hospital care for elderly joint replacement patients involves intense rehabilitation services like prescribed physical therapy exercises to improve strength and range of motion. This regimen continues at home after a stay in a skilled rehab facility. This is also a common need for post-operative home care to help maintain safety, consistency, and progress.
Our after-rehab caregivers can help with medication reminders, encouragement to keep up with prescribed physical therapy regimens, as well as meal preparation that supports healthy nutrition and hydration to promote continued healing at home.
Heart Surgery or Cardiac Events
You or your loved one deserve reliable after-hospital care services during the recovery period after a major procedure. Outpatient cardiac rehab may last several weeks and involves monitoring responses to exercise, plus education about cardiovascular risk factors. For those recovering from cardiac procedures, our team provides home care after heart surgery to support your provider's recommendations and daily routines.
Once back in the comfort of home, following this program is essential to get you or your loved one back to enjoying life. This will include follow-up healthcare provider appointments and a heart-healthy lifestyle that may include major changes to diet and exercise.
Thanks to our skilled caregivers, these are changes no one needs to face alone. We can help with medication reminders, meal planning and preparation, and running errands since driving is generally prohibited post-surgery.
Eye Surgery
Following eye surgery, vision may not be reliable or functional for some time. As part of post-operative home care, our care team can help with day-to-day tasks that you or someone you care for may not be able to do during the recovery period, including driving. We can also handle tasks to help eliminate the need for bending or lifting, which can put strain or pressure on the eye.
Pneumonia Prevention
The occurrence of pneumonia after surgery is not very common, but the risk still exists and is often a result of anesthesia and lack of mobility. Patients are usually advised to practice deep breathing and coughing exercises after surgery, which expand the lungs to help prevent pneumonia. Our caregivers support post-surgery pneumonia prevention by maintaining a clean environment and encouraging good nutrition and hydration for better overall health—key ways our skilled after-hospital caregivers can assist with pneumonia prevention after major procedures.

Please allow me to comment on Whitnie Hensley as a member of HOMEWATCH. She has been invaluable since she first came to my home. Whitnie's willingness to assist me in every possible way is commendable. She anticipates my needs almost before I do and she is proactive. It is clear to me that she has considerable experience and the empathy that makes her so qualified for her position. She represents HOME WATCH professionally and compassionately. I am most grateful for her assistance..
My family is incredibly grateful for the outstanding care and support provided by our at-home care manager, Kim Pierce, and our at-home caretaker(s), Mariah, Marisol and Tish, for my 100-year-old grandfather! From day one, they have shown genuine compassion, patience, professionalism, and attention to every detail of his needs. Managing care for a loved one at this stage in life can be overwhelming, but they have made the entire process smooth and reassuring for our family. They consistently go above and beyond to ensure my grandfather has been comfortable, respected, and well cared for. Their communication is excellent, and they treat him with kindness and dignity at all times. It is rare to find a care team so dedicated and trustworthy. Our family truly appreciates everything they have done, and we highly recommend Kim and her team of nursing aids at Homewatch CareGivers of Houston Galleria to anyone looking for exceptional home care support.
In addition to my current caregiver, Georgette who I would have welcomed as a daughter I have had outstanding results with Homewatch CareGivers of Yorba Linda. They're 5 star all the way! From the beginning of my recovery I have received help in bathing, medication, meal preparation and companionship. Most of them are pursuing advancement in the medical field and I hope I have encouraged this.
❤️❤️❤️Diana! is a great caregiver. She represents the company very well. She helps me with majority of my daily needs and make essential daily living easier as I require a little more assistance! She is very Thorough and friendly and patient centric
I want to give the biggest shout out to Kate A. with this company. My mom has a VERY unique condition that full service care facilities are not capable of serving, so we looked for in home care. Kate met with me and she said her company could not serve my mom and that she did not know of one that would (which turned out to be true as all 10 companies I talked to said they could not provide that specific care and they did not know of one that would). Rather than leave us to fend for ourselves, Kate showed a true compassion for the work she does and did some extra work to call around and try to find one company that might provide that unique service - even though her company would not be getting a dime from us. That one company ended up being able to help. Without Kate's extra effort done strictly out of care for the elderly, we might not have found the unique care we needed.
After my husband had two emergency surgeries in a week, I was extremely concerned about what would happen when he came home, because when he was given discharge instructions we each heard something different even though we were both right there. We did not agree on what was said or on the importance of actually doing it. He also made it clear that he did not want me to tell him what to do. Kimberly Pierce, our care manager, was a gift from God. She was a neurology ICU nurse for 16 years, and was able to explain what needed to be done and exactly why it was important and necessary. When I thanked her for helping us both understand and get on the same page instead of arguing and becoming upset, she joked that saving marriages was all just part of her job.
Excellent home health care service with compassionate caregivers and reliable support. The team was professional, attentive, and made a real difference in daily care and comfort.
We cannot say enough great things about Homewatch, Martin and staff. The caregivers are awesome and provided excellent care to my mother in law. Martin and Ellen worked tirelessly to assist us in increasing care requirements needed over a 7 month period and finding the right fit.
